📎 CONSENT & RELEASE FORM

Every client is required to read and sign this form before receiving services at We Fucking Love Your Hair (WFLYH) including cutting, coloring, styling, treatments, extensions, makeup, or other salon services. By signing, you agree to the following:

APPOINTMENTS

  • Automated Reminders: Sent 6 days, 3 days, and 3 hours before appointments.

  • Rates & Estimates: All services are gratuity-free and priced according to each provider’s individual rate. While we share cost estimates when you book, the final price is confirmed during an in-person consultation, which may take place at the start of your session or in a separately booked appointment.

  • Late Arrivals: Appointments are a reservation of time and priced at an hourly rate. The clock starts at the scheduled time, not upon arrival. Arrivals over 15 minutes late may result in appointment modification, with a possible late fee.

LATE CANCELLATIONS & RESCHEDULING

We kindly ask for at least a forty-eight (48) hours notice prior to canceling / changing / rescheduling any appointments. A credit card is required for booking, and by providing it, you authorize its use for any fees due to late cancellations or no-shows.

  • More Than 48 Hours’ Notice: No Fee

  • Less Than 48 Hours’ Notice: 50% fee of the total cost of services booked, charged to the card on file. 

  • Illness / Emergency (less than 48 hours' notice): 50% fee of the total cost of services booked, charged to the card on file. This fee will be credited toward your next appointment with the same service provider. Credits are valid for 12 months.  No need to explain—just let us know it’s an illness or emergency. Please text us at 312-344-3683 for any late cancellations.

  • No-Call / No-Show: 100% fee of the total cost of services booked & future booking privileges may be revoked.

💲 PAYMENT

  • Payment: Due at the time of service via card present, card on file or cash (exact change preferred; excess applied as a gift card).

  • No Refunds: Applies to services, cancellation fees, products, and hair extensions.

  • Gratuity Free: We are proud to operate as a gratuity-free salon, meaning tips and gratuities are not accepted after your service. The cost of your experience is already fully included in the pricing.

📷 MEDIA CONSENT

  • Photo & Video Release: We may capture images & videos of you and your hairstyle before, during, and after services. This may include close-up shots of your hair, overall looks, and transformation processes. These images may be taken for internal documentation purposes, including tracking hair history, formulas, haircut references, and service records to ensure consistency and quality in future appointments.

  • Public Use & Consent: Images and videos may also be used for commercial or non-commercial purposes, including advertising, social media, promotions, training, and website features. You have the right to opt out of public sharing at any time. If you prefer that your images or videos not be posted or used publicly, you must notify us in writing prior to or during your appointment. To opt out: please text, "opt out public use" to 312-344-3683.Opting out of public use will not affect your service in any way. Internal documentation for hair history and record-keeping may still occur.

  • Rights & Usage: By signing, you grant WFLYH and its affiliates the irrevocable, perpetual right to use, modify, and distribute approved images, videos, and related content for the purposes described above, unless you have formally opted out of public use.

  • Ownership: You acknowledge that WFLYH retains exclusive ownership of all media captured during your appointment. You agree not to contest these rights or request additional compensation for any approved future usage.

📃 LIABILITY WAIVER

  • Assumption of Risk: You acknowledge the risks associated with salon services, including chemical treatments, cuts, styling, and extensions, and accept them as part of your experience.

  • Release of Claims: You release WFLYH and its staff ("Salon Parties") from any liability for injuries, allergic reactions, hair damage, or unexpected results, including claims related to media use of your likeness.

  • Health & COVID-19: You confirm you’re in good health, free of illness symptoms, and will inform the salon of any medical conditions that may affect your service. You release the Salon Parties from liability for illness transmission, including COVID-19.

🔮 GRIEVANCES & ADJUSTMENTS

Service Satisfaction: WFLYH takes your hair seriously and we value open, good-faith communication. If you are dissatisfied with a service, please adhere to the following steps:

  • Notify us within 7 days at WFLYH@ifuckingloveyourhair.com for adjustments.

  • Agree not to post negative comments, reviews, or grievances about the salon on social media or other public platforms without first allowing WFLYH the opportunity to resolve the issue.

  • Legal Recourse: By signing this agreement, you acknowledge that bypassing this agreed-upon process may make you liable for damages, including but not limited to attorney fees resulting from unsubstantiated or harmful statements. The salon reserves the right to pursue legal action to recover damages if grievances were not communicated directly to us first or if false statements are made that harm the salon's reputation.

Policy on Complimentary Services: We do not offer complimentary services for changes in preference or direction once services have been rendered.

📦 MISCELLANEOUS

  • Governing Law: This release will be governed by the laws of the State of Illinois, without reference to its choice of law rules.

  • Severability: If any provision of this release is determined to be illegal, unenforceable, or void, the remaining provisions will remain in full force and effect.

  • Acknowledgment: By signing, you acknowledge that you have read, understood, and agreed to this form in its entirety.
